How to Apply for a Passport

How to fill the Form


The Passport Application Form is Machine Readable. Please follow the instructions as follows:

  • Use capital letters only. Particulars given in the form will be printed in the passport.

  • Therefore, please be careful in filling up the form and avoid any mistakes.

  • Use black/blue ball pen only

  • Do not fill the form with pencils or ink-pen.

  • While filling up the boxes, kindly leave one box blank after each completed word.

  • Write clearly within the box without touching the boundaries.

  • Adjust the information to fit within the number of given boxes.

  • Do not write anything outside the box. Avoid over-writing

  • Incomplete application will not be accepted.

Note: 

All original documents are to be shown at the time of submission of the passport application form. With the Original Passport Application Form, self-attested copies of all required documents need to be attached. Illiterate applicants should put only thumb impression in the box meant for thumb impression/signature. 

In case an applicant has stayed at more than one address during the last one year, he/she should furnish two additional photocopies of the PP form for each additional place of stay.

Fees Structure

The following all-inclusive fee is to be paid along with the application form, either by Bank Draft in favour of the Passport Officer concerned or in cash.

 In case of DD, full name of applicant and application number to be written on the reverse of the draft. 

The Bank Code of Issuing Bank, the Demand Draft Number and date of issue of the Demand Draft should be mentioned.

Details of payment of fee may be mentioned in the relevant boxes in the application form.

Where to Apply


For Online Registration, the applicant has to submit the print outs of the application form along with required documents to the respective passport office in the appointment date and time printed. Apart from this Passport Applications can be submitted at

  1. The counter of the Passport office
  2. The Speed Post centers
  3. District Passport Cells
  4. Passport Collection Centers

An application for a passport may be submitted personally OR through a representative (any close member of the family or recognized travel agent(s) only) carrying an authority letter.


DELIVERY OF PASSPORTS

Passport may be delivered only to the applicant or dispatched by speed post to the address given in the application form.

I need passport in a week’s time. How do I get it ?
You may apply in tatkal service on the basis of either a Special Verification Certificate or three out of Fifteen Documents prescribed by the Government along with standard affidavit Annexure I. Subject to all documents being in order, passport will be dispatched by speed post in a week’s time. For Tatkal service, you need to register online at www.passport.gov.in and then submit the application at the passport office in Secunderabad. Please note that presently Tatkal applications are not accepted anywhere else. You need to submit the application in person with all supporting documents, originals should also be shown at the counter.
Documents required for Tatkaal scheme ::
I. ANNEXURE I on non-judicial stamp paper attested by a Notary (mandatory for all Tatkal applications)
                                                                AND
II. Verification Certificate as per the specimen at ANNEXURE F (in two originals on official letterhead and photocopy of the ID card of issuing officer)
                                                                 OR
3 of the 15 documents mentioned below, provided one of the three documents is a photo identity document and atleast one of the three is amongst the documents indicated at (1) to (9)

Note: Application will be rejected if there is any discrepancy in the 3 submitted documents.
Does the applicant need to come in person to apply for Ordinary passport, Tatkal, Miscellaneous passport?
In all the cases, the applicant is required to come in person to submit the application form (except for minor applicants where at east one of the parents should be present). However, in exceptional cases (on health grounds etc) an authorized representative with identity proof my submit passport for ordinary or miscellaneous services.  The applicant also has the option to submit through Travel Agents authorized by the Passport Office. However, this does not apply for Tatkal cases.
What do I do if I have a Court case/ criminal case against me?
     As per the Passports Act, no passport is issued to a person if it comes to the notice of Passport Authority that a criminal case is pending in a court of law against that person. Passport is also not issued if a person is convicted in the last five years with imprisonment upto two years and above. However, Gazette Notification 570(E) dated 25th August 1993 permits issue of a short validity passport to a person against whom a criminal case is pending if he produces the permission of the concerned court for departure from India.
     Give full details in the appropriate columns in the application from. Further, obtain a Court order granting permission for travel abroad, which should be duly attested in original by the Court and enclose the same along with the passport application. If there is more than one case, the order should cover all the cases. Submit an undertaking to follow the Court's directions for travel abroad as per the terms laid down and to surrender the passport as directed any time by the authorities concerned.
     Please note that suppression of any material information relevant to the issuance of passport is an offence under the Passport Act, 1967.
I am a student staying away from my parents. Where do I apply for passport and what proof of residence shall I have to furnish ?
For example, if you are a student staying in Warangal and your parents are staying in Karimnagar, furnish along with your form, four Personal Particulars forms. Please also indicate your hostel address/the place where you are staying in Warangal as present address and your parent’s address as Permanent address.   In case you are studying and staying in Bangalore, you can either apply at the Passport Office Bangalore or in the Passport Office in whose jurisdiction your parents’ address (permanent address) is located, e.g., Hyderabad. In the later case, fill up an additional set of Personal particulars form giving the address in Bangalore. For proof of residence at your hostel/place of stay, you should provide either a Bonafide Certificate issued by the Principal/Warden, or, proof of residence of the guardian with whom you are staying along with the guardianship affidavit. In case your original certificates (SSC etc) are in the custody of the educational institution, you would also need to furnish a custodian certificate from the authorities along with photocopies of the certificates.
Till six months ago, I was working in Guntur. I have now shifted to Hyderabad. What documents do I enclose while applying for a fresh passport?
Mentioning your Hyderabad address in the present address column no. 9, you should also mention your Guntur address in column no. 9. a. where you are asked to furnish your address if you have been residing since less then one year.  You should also furnish two additional personal particulars forms indicating your previous address. This should be furnished along with other requisite documents.
I had applied for fresh passport three months back in Mumbai while I was working there but, did not receive my passport.  I am now urgently in need of my passport and my file is pending. Can I apply in Hyderabad, if so, how?
You may apply in Hyderabad by furnishing details of your Mumbai application in the relevant column of the form. Suppression of material information regarding your previous stay will lead to imposition of penalty of Rs. 5000/.
My passport was issued at Hyderabad 10 years back. I have been working in Delhi for last 5 years and moved back to Hyderabad 2 months back.  I now want to renew my passport. Where should I apply, whether in Hyderabad or Delhi ?
You should apply in the Passport office in the jurisdiction of your present address only. Addresses should be clearly mentioned in column nos. 9 and 9a. Proof of stay on present address should be furnished. In case no proof is available as per list prescribed by the Government of India, you may obtain a Residence Certificate issued by the Mandal Revenue Officer. Please note that period of stay and addresses should be mentioned as specifically as possible.
Damaged / Lost Passports::
I have obtained passport from your office one year back and lost it recently while traveling in the city. What are the procedures I should follow to obtain another passport?
You may report about the loss of passport at the Police Station   concerned and along with the loss certificate issued by them, you may apply for a fresh passport on form no. 1, enclosing copy/details of your lost passport.  If there is any change in your present address / any other particulars, requisite documents should be furnished as prescribed by the Government of India.
My passport got damaged during the heavy rains few months back. How should I get a new one ?
You may apply for a new passport by submitting form no. 1. You should furnish your damaged passport at the time of submission.  If there is any change in your present address / any other particulars, requisite documents should be furnished as prescribed by the Government of India.
Fee for valid lost / damaged passport is Rs.2,500/.
Fee for expired, lost / damaged passport is Rs. 1,000/.
I have obtained passport from Hyderabad office 22 years back, which is misplaced and its details are not readily available with me.  I am in need of a passport to visit my son in the US.  Please advise me how to proceed in this regard.
You may apply by submitting form no. 1 enclosing Loss Certificate issued by the Police authorities of the jurisdiction concerned.  Passport will be issued on receipt of clear police verification report.  The application should be submitted in ordinary service and not Tatkal.
Minors:
What are the documents required to obtain a passport for minor?
  • Annexure H duly signed by the parents
  • Birth Certificate issued by authorities concerned under RBD Act
  • Parents’ passports
  • Address proof if it differs from parents’ passports
  • In case of either parent’s consent not available, affidavit as per Annexure G
  • If one parent is abroad, Sworn Affidavit duly attested by Indian Mission along with an affidavit from the parent residing in India
  • Three latest passport-sized color photographs with a clear background
  • Affidavit as per Annexure ‘C’ to be submitted with the application for a passport of a minor child by either parent (who are separated but not formally divorced) or by a single parent of the child born out of wedlock
  • In case the parent applying is a divorcee, furnish Court Order giving custody of the child.
My child is five years old. Should I affix his thumb impression or signature on the application form ?
As the child is small and the handwriting is not likely to be stable, you may affix his thumb impression in the relevant column (box specified for signature or thumb impression of the applicant on first page of the application).  The parent should counter-sign in the declaration column of the application (i.e. column no. 19 in fresh form (EAP-1) and col. no. 11 in miscellaneous form (EAP-2).
My child is 16 years old.  What is the fee I have to pay for a fresh passport ?

To ensure that every birth is duly registered with the Registrar of Births and Deaths under the RBD Act, the Government of India has made it mandatory to submit Certificate of Date of Birth even though the applicant has a Secondary School Certificate for all applicants born on or after 26.01.1989.
I am a single parent having an adopted child.  What are the documents I need to submit for obtaining a passport for my child ?
  • An Adoption Deed executed under the Hindu Adoption and Maintenance Act.
  • Date of birth Certificate (if available, or Court declaration for the child’s date of birth)
  • Annexure ‘H’
  • Your passport
  • Proof of address if it differs from your / spouse’s passport
  • Three latest coloured photographs with clear background.
  • Annexure I if required in Tatkal
I have a one-month old baby.  Does the baby require a separate passport, or can his name be endorsed in one of the parent’s passports ?
Minor’s names are not endorsed in their parents’ passports anymore.  A separate passport has to be obtained for minors too.
Miscellaneous services::
NOTE: For all miscellaneous services like inclusion / deletion of spouse name, correction in personal particulars, change of address, renewal of short validity passport (a passport issued with short validity viz., 6 months, 1 year, 5 years etc), PCC, ECNR etc, the application has to be submitted at the Passport Office only. Online registration is not required. Form No.2 is required for PCC and ECNR, and for all other services Form No.1 should be submitted.
Police Clearance Certificate::
What are the requirements to obtain PCC ?
  • A valid Indian passport
  • Application form no. 2 duly filled in
  • Copy of the passport
  • Fee of Rs. 300/.
      If Police Verification on your previous file was not received/ adverse or incomplete, verification must be conducted again before PCC is issued.
My passport was issued in New Delhi three years ago and I am presently staying in Hyderabad for last two years. I need PCC for Canada. Can I apply at RPO, Hyderabad ?
Yes, you may apply by furnishing the following documents:
  • Valid Indian passport
  • Application form no. 2 duly filled in
  • Copy of the passport
  • Fee of Rs. 300/-
  • Proof of present address
  • Two personal particulars forms
Emigration Check Not Required (ECNR)
Who is eligible for Emigration Check Not Required (ECNR) endorsement on the passport?
  • All holders of Diplomatic / official passports.
  • All Gazetted Government servants.
  • All Income-tax payers (including Agricultural Income-Tax payees) in their individual capacity.  Proof of assessment of Income tax and actual payment of Income Tax for last one year or copy of PAN card to be insisted upon, and not merely payment of advance tax.  However, in most cases as assessment order is not issued separately by the Income tax department.  Income tax returns which are stamped by Income tax authorities can be accepted.
  • All professional Degree holders, such as Doctors holding MBBS degree or equivalent degree in Ayurved or Homeopathy, accredited Journalists, Engineers, Chartered Accountants, Cost Accountants, Lecturers, Teachers, Scientists, Advocates etc.
  • Spouses and dependent children of category of persons listed from (b) to (d) above.
  • All persons having qualification of Matriculation and above.
  • Seamen who are in possession of CDC or Sea Cadets, Deck Cadets;
  • who have passed final examination of three years B.Sc  Nautical Sciences courses at T.S. Chanakya, Mumbai and
  • who have undergone three months pre-Sea training  at any of the government approved Training Institutes such as T S  Chanakya, TS Rehman, TS Jawahar, MTI…………
  • Persons holding permanent Immigration Visa, such as the visas of UK, USA and Australia.
  • Persons possessing two years diploma from any Institute recognized by the National Council for vocational Training  (NCVT) or State Council of vocational Training (SCVT) or persons holding three years’ diploma / equivalent degree from Institutions like Polytechnics recognized by Central / State governments.
  • Nurses possessing qualifications recognized under the Indian Nursing Council Act, 1947.
  • All persons above the age of 50 years.
  • All persons who have been staying abroad for more than three years (the period of three years could be either in one stretch or broken) and their spouses.
  • All children upto the age of 18 years of age (At the time of re-issue at the age of 18 years, ECR stamping shall be done, if applicable).

IMPORTANT: No emigration clearance is required for visiting Bangladesh, Pakistan and all European countries (excluding Commonwealth of Independent States (CIS) states), North America, Australia, Japan, New Zealand, South Korea, South Africa and Singapore.
My passport is issued by PO, Madurai. I am residing in Hyderabad since two years. Can I apply for ECNR here?
Yes. Provided, you fall under any one of the categories from a. to m. of question 1 under ECNR mentioned above, and you have proof of stay on your present address in Hyderabad.
I don’t have ECNR stamp in my Passport. Can I get it on the basis of my provisional matriculation or above certificate issued by the University/Board concerned?
We are no longer affixing ECNR stamp in passports. In fact it is not even printed in the new passport booklets. Currently, only ECR stamp is being put in passports if applicant is not eligible for ECNR. However, you are entitled to ECNR status if you possess qualification of Matriculation or above. Even you can get ECNR status on the basis of provisional matriculation certificate if the mark sheets of all semesters of your course are available with you.
Address change ::
I got my passport from Delhi. Now I have shifted to Hyderabad. Should I change my address in the passport?  If change is required, what are the documents to be furnished and how much time will it take?
It is up to you to change the address. It is always better if your passport indicates your current address. For this purpose, you need to fill up form-1 and submit it along with valid proof of your residential address at Passport Office of the area of where you reside.
Change of spouse name ::
I am holding a passport which includes my husband’s name. Now that I am divorced, how do I get my husband’s name deleted from my passport ?
You may apply for a new passport under miscellaneous category requesting for deletion of spouse name. Divorce certificate issued by a competent authority should be submitted along with the application form. In case of change of address or any other particulars, the requisite documents may be furnished. In case you would like to change your surname subsequently you need to submit (i) Divorce deed duly authenticated by Court and (ii) Deed Poll or Sworn Affidavit as per specimen in Annexure –D on non-judicial stamp paper attested by a Notary.
Corrections in passport::
If there is any mistake in the personal particulars of the applicant in his/her passport, the same may be submitted at the Passport Office for correction at the Correction Counter. If the mistake was made by the applicant in the original application form, then he/she would have to apply for the correction in category ‘R’ by submitting form no. 1 and a fee of Rs.1000/ would be charged. Online registration is not required in this case.
I am holding a passport in a particular name say Annam Sujatha, now I want to include my father’s surname and change my name to Annam Sujatha Reddy.  What additional document is required ?
For any change of name even an alteration of one alphabet, the applicant should submit Deed Poll affidavit and two original newspaper cuttings announcing the change of name published in two leading daily newspapers (one daily newspaper should be of the area of the applicant’s permanent address and another of his present address)
I am holding a passport and want to change my father’s/mother’s name. What documents are required ?
In this case, the advertisements in the newspapers have to be given by the father/mother whose name is to be change and even the deed poll affidavit has to be signed by him/her.  Documentary evidence regarding changed name, be enclosed.
I am a divorcee and remarried. Now I am applying for the passport of my minor child. I want to include my present spouse name in his passport.  What are the documents required?
In event of remarriage after divorce, the biological parent’s names will only be entered in the passport.  However, if the stepfather/stepmother is appointed by the court as a legal guardian, the name of such stepparent can be written as legal guardian. In the event of remarriage after death of the spouse the name of stepfather/stepmother may be written in the passport against the column of father/mother.
I want to change my date/place of birth in my passport. What documents are to be enclosed?
     When a competent authority which issued a birth certificate (Municipal authorities) or SSC certificate, issues any correction or amendment in date/place of birth, no court order is required (if the same document was submitted earlier for the passport), However, fresh fee will be charged and an affidavit to that effect should be given. In case there is a conflict between the birth certificate and the SSC, birth certificate usually prevails.
     Supporting documents submitted at the time of your old application would be compared with the documents submitted with the new application. A declaratory order from a First Class Judicial Magistrate of a Civil Court may be required. Please note that suppression of information/ providing of wrong information is an offence under clause 10(3) (b) of the Passport Act.
I have obtained passport 5 years ago and now as my appearance/signature has changed, I want to take a fresh passport. What documents are needed ?
In case of a valid passport for more than one year, a sworn affidavit has to be given for change of appearance.
In case of requests from Sikhs who want to change from turbaned photograph to clean shaven ones, or any other major visible change in others, a notarized statement or certificate from any of the authorities authorized to sign verification certificates should be submitted. Fee will be charged for a fresh passport.
